| More details: | Care Wisconsin's Day Center is committed to help loved ones remain independent and living in the community as long as possible. Our adult day center provides quality care in a safe and supportive setting where clients can actively enjoy the day. The center offers a unique continuum of care, easing transitions as individual needs change. Connections day center program is designated for adults who have medical conditions, physical disabilities, cognitive concerns, or difficulties functioning at home alone. The Gathering Place program is exclusively for adults with Alzheimer's or other memory loss conditions. We develop personalized care plans to tailor services and activities to individual needs, interests and abilities. Care plans, written with family involvement, are reviewed regularly to evaluate outcomes and goals. Hightly trained staff provides health monitoring, assistance with medications and support with feeding, toileting, mobility and nail care. We serve nutritionally balanced meal and snacks daily. We also offer specialized Programming for people with Memory Loss. The Gathering Place program customizes its activity programming based on the level of memory loss and other needs. Programming is designed to maintain and encourage cognitive skills for as long as possible. Client's receive a full day of activities to help maintain regular daily routine and sleeping pattern. Each day's activities are scheduled to fit the rhythms and behaviors of individual clients. |
